SCHEDULE OF VISITS

SCHEDULE OF VISITS - BIRTH TO 21 YEARS:

The first newborn visit should occur between 5 and 7 days of age or earlier if recommended by the hospital in order to follow up jaundice (discoloration of the skin and eyes) or any other problems.

A typical schedule of well visits is as follows:

  • At 2 weeks of age - monitoring feeding and weight gain;
  • At 1 month of age - measurements, development and Postpartum Depression Screening;
  • At 2 months of age - measurements, development, vaccinations and Postpartum Depression Screening;
  • At 4 months of age - measurements, development, vaccinations and Postpartum Depression Screening;
  • At 6 months of age - measurements, development, vaccinations and Postpartum Depression Screening;
  • At 9 months of age - measurements, developmental milestones (ASQ-3);
  • At 12 months of age - measurements, development, vaccinations, blood test;
  • At 15 months of age - measurements, development and vaccinations;
  • At 18 months of age - measurement, developmental milestones (ASQ-3), Autism test (M-CHAT) and vaccinations;
  • At 24 months of age - measurement, development, vaccinations, blood work, Autism test (M-CHAT);
  • At 30 months of age - measurement, developmental milestones (ASQ-3);
  • At 3 years of age - measurement, development, vision and hearing;
  • At 4 years of age - measurement, development, vision, hearing, vaccinations and  blood work;
  • At 5 years of age - measurement, development, vision and hearing;

After the age of 6, children and adolescents should be seen for their annual physical at which vision and hearing are assessed and tested if there is parental concern.

  • At 11 years of age - measurement, hearing, vision and vaccinations
  • At 12-17 years of age - annual Risk Assessments: Smoking/Tobacco Questionaire, Screening for Alcohol and Substance Use and Adolescent Depression Screening (PHQ-9)
  • At 16 years of age - measurement, hearing, vision and vaccinations
  • At 18-21 years of age - annual Risk Assessments: Smoking/Tobacco Questionnaire, alcohol use screening (AUDIT-C), substance use screening (DAST-10) and Adult Depression Screening(PHQ-9)

Everyone should get Tetanus vaccination at least every 10 years.
